I can give you a general idea based on my knowledge and experience in the State of California/Nevada.
On your document, you can state that you may wish to choose an alternate but that you wish for your son to be the deciding vote. You can then have this document notarized in front of a notary with witnesses. You can fax said document to your son in Virginia for him to sign in front of a notary with witnesses - and request that he fax it back to you - however, he needs to mail back the hard copy, with his original signature.
Note: At the bottom of your document, you can add the following: I _________(your name) hereby approve the fax signature of ________(your son's name) to my "Durable Living Will" along with an attached original signature of ________(your son's name) whereby this document is deemed valid. Confirm with adding your signature and date.
I would suggest that you contact a paralegal's office to assist you in the preparation of same.
