How do you file a lawsuit? What are the guidlines?

I would like to file a lawsuit against UNUM Provident Life Insurance Company. What do I need to put in the letter to the court to file for this lawsuit? Today marks the 2 year anniversary of the declination letter, so I need to do this today.
