Insurance coverage for the child

it was court order that the child is covered by the father's insurance, his last name was amended to the birth certificate. This is the question. If the last name of the child is not the same of the biological father, would that be the reason way the insurance carrier would not pay for her vision,dental and or health(last name difference)

It should not make a difference about the last name. If the child is a confirmed dependent of the father, and with the court order, the child should be covered by the insurance company and therefore the insurance carrier must pay; or the father must get insurance that will. Providing them with the birth certificate and the court order should be sufficient for them to pay for her coverage through the father.
