my husbands father passed away 1 month ago. his annuity paper for beneficiary ask for his fathers address. We are unsure what to put since prior to his death he moved back to new york for better care. he was living in florida for a yr. But his death certficate shows his new york address. should we use that? or the florida address where his will was done?

I would use the New York address, but if there is room, but the Florida address too. Maybe even attach a second page in case their system is set up with the Florida address.
