Do I need a permit to send fruit via Federal Express from Los Angeles California to Tucson Arizona? This is fruit from the tree in my own backyard. If so, where do I get the permit form?

As long as it is a small personal box of fruit, it is considered non-commercial, and you would not need any special permits.
